About The Company 


Our client is a small, stand alone not for profit aged care community facility providing residential care in beautifully presented apartments. They provide care for their residents who are aged between 55 and 100 years old. Many of their residents are still active members of their local community and our clients' philosophy is to encourage their residents to maintain their independence for as long as feasible. There is always a busy hum around the facility from either the dining room serving lunch, morning and afternoon tea or from the busy activities schedule they have. 

About The Opportunity

They are looking for a Clinical Care Coordinator to join their company in Chatswood, NSW, with the potential to earn $84,000 per annum (prorated)  + Superannuation.

The primary purpose of this position is to manage and lead the delivery of person centred care of residents, ensuring the highest
standard of ongoing assessment, care planning, and evaluation and clinical governance.

Reporting directly to the Clinical Care Manager some of your responsibilities will include:

  • Ensuring the delivery of quality clinical and person centred assessment for residents
  • Overseeing the coordination of ACFI assessments
  • Delivering complex healthcare to a range of patients
  • Managing medication inclusive of Schedule 8 drugs
  • Leading the team by providing supervision, coaching, feedback, and support
  • Helping with rosters and the continued improvement of the organisation 

About You 

The successful candidate will have:

  • Current Registered Nurse registration with AHPRA
  • A minimum of 5 years experience as a Registered Nurse in aged health care
  • Experience in clinical governance, accreditation, and ACFI
  • A friendly, professional and organised nature and approach to work
  • Strong leadership skills with excellent written and verbal communications skills
  • The capabilities to make high-level decisions
